I have a feeling you might be storing the images inside the CI system or application folder so the .htaccess is blocking them from being served since those are protected directories.
when I try to access those images, I get the default Codeigniter 404 error page

@Matt CI should not have to access anything in there for the most part unless it's say storing cached images or something. Those should be accessible to the browser. Make sure the paths are correct and match your structure.

Codeigniter, being a framework in PHP, also has those powers.

@Matt change the 2nd line to this RewriteCond $1 !^(index\.php|images|smileys|robots\.txt) you may have to add the ci path as well. This is why you should have an assets folder so all your browser accesible files you put in there and just exclude that one folder in the rule.
