If I was starting again, I would choose Colemak instead of Dvorak. But, since I learned Dvorak more than a decade ago (when Colemak didn't exist), I didn't have the choice then.

Definitely worth the switch. But as I mentioned, I would recommend to choose Colemak over Dvorak if you're starting now.
 
Since I have a keyboard that natively supports Dvorak, I'm not sure what I'd gain from yet another layout.

I think it was 2 weeks before I stopped being dog-slow and looking at the little crib-sheet all the time.

do NOT rearrange the keys. if they still say "qwerty" on them, and looking at the key labels is useless for you, then you can completely break the habit of looking at the keyboard. this is good!
also, if you find yourself in BIOS and needing to find keys like Y, N, Q ... having the qwerty labels can sometimes actually be a life saver :)
