login tokens, email confirmation tokens, password reset tokens, etc.
Code checks to make sure they aren't expired but they just leave em there
I normally just stuff them into redis with an expiry
For long lived tokens, you can clear them on some valid action (logging out -> delete auth token, email confirmed -> delete confirmation token)
yeah but what if email isn't confirmed
And have a cronjob or something to clear up tokens with no action after 24h or so (can be less, depends on the security level you want)
We clear up some of the more sensitive tokens every 45 minutes

totp/hotp i think is the most realistic solution
I'm not saying to only offer hardware token, offer all three if you want, but don't make it dependent on SMS - someone should be able to set up totp/hotp without sms
A lot of sites force you to set up sms anyways, which is the weakest link here
Totp/htop is a good measure that covers most people reasonably well
And is easy to use nowadays
Its easier to implement than sms anyway
sms you gotta setup twillio or some shit
totp/hotp you install a simple lib and store some keys and you are done
