A: Laravel 5 localhost installation not working

I don't have those lines in my public/.htaccess file.
Get the default .htaccess from the laravel package: github.com/laravel/laravel/blob/master/public/.htaccess comment those lines and move it to the public folder.
Sorry, I did have those lines. I commented them out as you said, but it did not make a difference.
Try this other .htaccess from laravel: laravel.com/docs/5.0/installation#pretty-urls
That also does not work.

I use a different php install
I can recommend it: php-osx.liip.ch
do whatever just come back with mcrypt
Just run: curl -s php-osx.liip.ch/install.sh | bash -s 5.6
it doesnt matter but when new php comes out what next?
how can you easily upgrade?
You remove the php folder it creates and the symlink
and you're back to default
ye or in my case just brew upgrade and I am done

If you still get a 500, check the .htaccess stuff I posted in your question. I just created a new laravel installation and got blank screen. Got into my working project, copied the .htaccess to the new project and it worked.
I ran brew install homebrew/php/php55-mcryptand it went through a really long installation.

you have the default php installation somwhere and now you have a different php installation, you need to swap which one apache loads
