maybe I'm doing something wrong but when I click the button it doesn't add another field, same with vector's script as well and the script I posted in my question.
Do you have Javascript disabled?
Also if you're copying this into your project, you'll need at add a <script> tag linking to jQuery > 1.7 in the HEAD.
If you run into problems like this with Javascript, just whip open the Javascript console (in Chrome, or download Firefox's firebug) and it will tell you what's going wrong.
no JavaScript isn't disabled I use it to calculate the totals in my form so I know it's working in the browser, I just tried what you suggested in firefox and nothing happens, no errors in firebug

my bad, every time I click the button to add a new field in the script above it adds an error to firebug but I can't see where the error is at. give me a minute I'll upload it to a server

For a start change all your javascript keywords to lowercase. function instead of FUNCTION etc
Then make sure the function is defined in the scope you're calling it. Or add it to the window object: jsfiddle.net/69Fvp

that's awesome, exactly what I want, BUT, lol, I need those new fields to fall inline with my form so the next row would be item_qty2, manuf_2, part_number2 and the next field to be item_qty3, manuf_3, part_number3 on down to a max of 5 so no more than 4 fields being allowed to be added. here is the form I'm using now jemtechnv.com/development/beta/finish_ticket.php?id=37 where there are 5 rows I want to reduce down to 1 row with the option to add what they need up to 5.
Matt maybe this option would be easier since my rows are already using a javascript to total up the quantity, price, hours and rate what if I was able to make the rows hidden and if they need them they can mark a check box to display the field? is that possible? if so how will that effect how my update query works? will I still be able to add ALL the values and not have a syntax error?

can it be made to limit the addition of fields to only 5? and what about removing fields if too many are added? or am I making this too complicated and should just leave the form as it is and figure out how to get the update query to skip over any value without an input?

It does limit it to 5. Study the code I posted and try to understand what each part is doing. It will then become clear how you can modify to do anything you want.

Matt, so I made some changes and have this script now jsfiddle.net/vegas/2gPfn but for some reason I can't get it to work in jfiddle, it works fine in a browser though my only issue is limiting it to 5 rows added and how do I get the script to change the field name for the input of each new row, example my existing row in the form is item_qty1, manuf_1, part_number1, part_description1, part_price1, and price_extension1 the next row I would need the fields to change to 2 and so on
Actually Matt I will ask that question of changing the value names in stack overflow I don't want to bother you with endless questions. Thanks for your help!

The reason it doesn't work in jsfiddle is because jsfiddle will wrap your javascript in a closure. This essentially creates a non global scope for objects thus when you call addRow in your onClick handler it can't find such a function in the global scope. This is a good thing because you ideally don't want to pollute the global object with all your functions. The quick workaround is define your function as window.addRow() where window is the browser's global object.
A cleaner solution though is to avoid onClick handlers and use jQuery to bind click events to functions within your javascript using the jQuery on() function
