A: auto approval for environment in github actions workflow does not work

sorry but the suggestions do not help resolve the issue. 1. I'm using the latest version of the workflow i.e v1.0.6. I also approved item4 manually yet item1 and item3 remain unapproved ... does not get auto-approved. The token has permissions to perform everything and the approver mohtashims is also good.
the issue is reproducible by simply using my workflow. Also, if I list item4 in environment_allow_list it does not get auto-approved
@Ashar I understand you are using 1.0.6, but with node 20 instead of the node 16 used by the action (in case that matters).
i dont know how to force activescott/automate-environment-deployment-approval to use node.js 20 as it is third party or if you can suggest some other solutions?
@Ashar No, I was more suggesting for your workflow to install and use Node 16 instead of 20.
@Ashar In your job execution, the script does not add item2 and item4 to the matrix because they do not match the *_* pattern used in the if condition. If you intend for item2 and item4 to be part of the matrix as well, you would need to adjust the logic accordingly. For example, if all items need to be included, you could simply add them directly to $unique_envList without the if condition.
but i see all three in the matrix as seen in the snapshot.

Could there be a delay?
Sometimes, there might be a delay in GitHub Actions reflecting the state changes, especially if there is a high load on GitHub's servers. That could mean that approvals have been sent but are not immediately visible in the GitHub UI.
I would still check the token permissions (but you might already have done that, since you told me before it had all the permissions)

@VonC I don't think Could there be a delay? and this issue is unrelated to any GitHub incident. My runs are 3 days old and aren't waiting on anything. Would it be possible for you to copy my workflow and replicate the issue?
