I reviewed this as "Looks OK", but the consensus seems to be going the other way.
It seems pretty understandable to me, and the issues seem to be fairly clear (using kebab-case, not camel-case), not knowing how to attach images to inputs. There is also a correct looking answer, albeit code only, on the question.

Should I have reviewed this differently?

I have just discovered that I've been banned from Review queue for next 16 days, and the reason is specified to be incorrectly tagging "Requires Editing" during a triage.

It's just been 5 days since I've achieved the review privilege and I did read through the [guidelines](https://meta.stackoverflow.com/questions/295650) before starting on reviewing.

The [triage item](https://stackoverflow.com/review/triage/26166656) for which I seem to be getting banned had 3 out of 5 responses as "Requires Editing".

stackoverflow.com/review/suggested-edits/26180367 one of my latest edits, got rejected, i'm wondering why did it happened. I added the "Python" tag relevant to the matplotlib but got rejected, felt weird.

Most people following the main Python tag to answer, without the tag, they won't be able the see it, so it makes the edit important, i still didn't get it
I can't speak for post author, but I noticed an image that wasn't inlined. And and "i", that maybe should have been capitalized. I would probably have chosen to given the URL a title as well.
But ultimately the post author rejected your edit, so they don't seem to want the tag there.

@yagizcandegirmenci You're welcome :) Also, if a post is closed, you may get an edit rejected just due to that if you edit anything in the body, since edits make the post go to the reopen queue. And usually posts are closed because the author needs to improve their post in a way that an editor cannot.

@SamuelLiew another bug in ReviewQueueHelper: when reviewing a post's suggested edit by clicking edit (1), the userscript makes the radio buttons become bigger and numbers (1, 2, 3, 4, etc.) are not present, although keyboard shortcuts work.
