we have datadog hooked up to pagerduty, which sends the info out to whoever is on call

sadly it does not actually page them

if you have the budget for it, I'd recommend datadog. It's a nice RRD-style tool, but much easier to use than Zenoss or Nagios, and hooks up to a bunch of alerting tools.

especially if you're trying to monitor a bunch of ephemeral instances of a distributed app like GAE runs

if your cloud provider has monitoring tools, use them, otherwise your options are DataDog, New Relic, or spend 6 months hacking Nagios/Zenoss into shape
