But if you know me, you also know that I'm a 19 year-old guy with a 12 year-olds mind who likes fart jokes and even when working with serious stuff, still makes fart jokes instead of discussing specifics regarding that "serious" stuff. So, I'd obviously add some spice into the boring doctor bullshit.

My trouble is, I am thinking that my controller creates the models and views etc. Its hard to think of injecting them, especially if they are only conditionally required (based on the controller logic).

I understand ... I dealt with the same issue. I've essentially reworked my controllers so that they are injected with the exact domain model objects (data mappers, actually) and templating objects (when needed) in their constructors

The only time I'm okay with injecting the Provider into other classes is when the Provider is a composition element for a class that needs to instantiate a class it doesn't yet know about. A top-level routing class, for example. In Artax, the whole point of the Router is to instantiate one of an infinite set of possible controller classes.
 
The controller shouldn't directly create those objects, you should at least be using a factory

Let's say I get stdClass objects from a source or push it into something else, but within my code I'd like to use concrete types.

where layer crossing is when one layer knows about the layer above it, or multiple layers below it (anything but the imediate layer below)
