I must say I have never had cause to use bitwise operators, but I am sure there are some operations that I have performed that would have been more efficiently done with them. How have "shifting" and "OR-ing" helped you solve a problem more efficiently?
What are the chances of two sql queries being inserted at the same time with users having two different connections? Assuming they clicked at the same time but have different internet connections.

If your questions is "is it ok to not handle that error condition" than I'd go with yes, provided you're fine with providing simply an error page on the inevitable exception

@Axel Yes. One will always be the first to acquire the lock on the index (or table depending on the DB). That query than wins, even if it might be slower to process than the other query.
