I just heard about a very popular package for a meteor package that someone snuck some porn into - the github owner was working on some other project for months and didn't respond to remove it... Some guy ended up losing is job because he sent his company's director over to look at the tech to see if they wanted to use it in their stack... and... yeah - he got fired for that.
