they get substituted for direct instructions

if you can use those macros to access I/O immediately, why do people still go through the hassle of writing a device driver for I/O?
 
Check the man page, you can't "just call" that. You have to request kernel permission for your process first
e.g. the kernel needs to delegate that to your process
