I had to deal with this furnace that went over the red pressure line today. When it was clear I could no longer dampen the heat source, I retired to a safe distance and waited for it to blow.
I see that it indeed did blow up!

The pattern is clear: OP isn't asking What can I do to improve? but instead follows the route This shouldn't have happened. Point is: it already did. Trying to get them to improve stuff they tend to stick with undo what happened...

An in-law family member always complains about the trouble they have with neighbours. They've moved a lot, and everywhere they always got the most troublesome people next door!
You'd hope that at some point it might click that all those neighbours are now very relieved the in-law has moved elsewhere.

I had similar experience between two teams in different cities. Each team accused the other team they were mere morons. It stopped when we decided to bring the teams at a central location...

Once upon a time I got a flag declined. I thought I was right. I googled "stackoverflow sucks". What came up was so stupid that I figured that if the arguments as to why Stack Overflow suck are so moronic, it must be that Stack Overflow does not suck after all. I was still pissed because it all seemed sooo evident that my flag was right.. Luckily, I read some stuff on meta that explained it, saw the wisdom of it, and I adjusted my thinking. Some are not so lucky.

When did " declined - questions older than 60 days cannot be migrated " come into force for flag reviews? I can't find this policy anywhere.

Now I think we should have a policy as well for flags which have been sitting around in the queue for 60 days as well :) Just have them sort of disappear
 
I believe there is something like that. I don't know what the exact conditions for it are though
 
I found one from Feb 5 for me. It's trapped for a good long while I imagine
